In order to file a copyright infringement notice, you will need to submit a written communication that substantially includes the following (see your attorney or Digital Millennium Copyright Act Section 512 (c) (3) to confirm these requirements):

Pursuant to DMCA Section 512 (f), anyone who knowingly claims that certain material or activity is in violation of the law may be subject to criminal liability for damages subsequent to such claim.
